I work with you to implement your NDIS plan so you can pursue your goals, access the right supports and services, and build confidence to manage your plan with increasing independence. The level of coordination support provided depends on your needs and what your NDIS plan funds.
Level 1 Support Coordination - Support Connection
Purpose:
With Support Connection (Level 1) I help you to understand your NDIS plan and start connecting with the right supports and services. This level is designed for participants who are new to the scheme or need light‑touch support to get going.
What This Includes:
Explaining your NDIS plan, budgets and categories so you understand what your funding can be used for.
Helping you find and connect with NDIS providers, community services and mainstream supports (like health, housing or education options).
Assisting with initial bookings or introductions to providers.
Building your confidence and skills to use and navigate your NDIS supports more independently.
Offering basic help with communication between you and service providers to get things started.
Goal:
To build your skills and confidence so you don’t need ongoing help just to connect with supports.
Level 2 Support Coordination - Coordination of Supports
Purpose:
With Coordination of Supports (Level 2) I provide practical and ongoing help to put your supports in place, solve problems, and increase your ability to use your plan towards your goals. This is the most commonly funded level of support coordination.
What This Includes:
Helping you understand and implement your plan to achieve your NDIS goals.
Coordinating and organising a mix of supports and services so they work together effectively.
Connecting you to the right providers and supports that match your needs, preferences and goals.
Supporting you with service agreements, bookings and schedules.
Problem‑solving issues as they arise (e.g. if a provider isn’t meeting your needs).
Building your capacity to make choices and manage your supports more confidently over time.
Helping you prepare for plan reviews and adjust supports as your needs change.
Goal:
To coordinate your supports so they help you live more independently, maintain relationships and participate in community life, while you gain skills to navigate your plan with confidence.
